UnitedHealth Group spent less of every premium dollar on care in the third quarter and raised its 2026 profit floor, but a $610 million reserve release did part of the work. The company’s medical care ratio, the share of premiums paid out for care, fell to 88.4% from 89.9% a year earlier, and adjusted earnings reached $4.69 per share versus $2.92. Revenue was $112.7 billion, slightly below $113.2 billion a year ago. UnitedHealth now expects 2026 adjusted earnings of at least $20.50 per share, up from $19.50 to $20.00 in its July outlook. The 88.4% includes $610 million of favorable development on prior-period reserves; the second quarter’s 86.7% included $860 million, so the ratio rose sequentially as the cushion shrank. UnitedHealthcare’s Medicare Advantage membership was 7.605 million at September 30, up from 7.565 million at June 30 but down from 8.435 million a year ago. Optum Health, the physician and care-delivery arm, posted $22.9 billion of revenue, down 10%, with operating earnings of $735 million, a 3.2% margin versus 0.2% a year ago. Optum Rx scripts fell to 393 million from 414 million. For health systems and physician groups, the signal is that the largest payer has repriced and shrunk its book to restore margin, and Optum Health is still contracting. The release excerpt we reviewed does not restate full-year medical care ratio guidance, so the July range of 88.1% plus or minus 25 basis points is not confirmed as current. Confidence: High on reported figures. Sources: UnitedHealth Group third quarter 2026 8-K earnings release, UnitedHealth Group second quarter 2026 8-K earnings release.

Alignment Healthcare followed through and sued the Centers for Medicare & Medicaid Services (CMS) over its 2027 Medicare Advantage star ratings, putting the 2028 payment hit above $200 million. Our October 12 issue reported the insurer’s 8-K saying it expected to sue. Per Healthcare Dive, the complaint in the U.S. District Court for the District of Columbia argues the star ratings methodology is unlawful because the measures were set in technical notes rather than through rulemaking, and seeks to vacate the 2027 ratings and force a recalculation before the 2028 selling season. Healthcare Dive reports that Alignment asked CMS on October 6 to drop the contested measures and CMS refused October 7, and that the share of Alignment members in plans rated 4 stars or higher falls from 98% to 25%. Alignment serves roughly 240,000 members in five states. The question for the rest of the Medicare Advantage market is whether a court will treat the stars methodology as a rule that needed notice and comment, which would reach every plan’s quality bonus. Confidence: Medium. The complaint itself could not be opened this run, so the court, dollar and membership figures rest on Healthcare Dive. Sources: Alignment Healthcare 8-K. Suki, which makes artificial intelligence (AI) note-taking software for clinicians, raised a $90 million Series E to build agents that finish the work after the visit. Healthier Capital, Crosslink Capital and Hercules Capital joined existing investors, per Healthcare Dive; valuation was not disclosed, and Suki says it has raised more than $250 million across five rounds. The agents would handle referrals, prior authorizations for new medications, orders and follow-up scheduling. Suki’s technology is already embedded in athenahealth’s Ambient Notes and Zoom’s telehealth documentation feature, and Optum is a partner. For provider finance teams, the pitch is moving ambient AI from documentation into the revenue-cycle and access work that sits behind it. Confidence: Medium. Healthcare Dive is the only source reviewed; a company release was not found. Supplemental: Healthcare Dive on the Suki round.

Centene will redeem its last $568.7 million of 4.25% notes due 2027 on October 23. The insurer told the Securities and Exchange Commission (SEC) on October 8 that it delivered a redemption notice at 100% of principal plus accrued interest, leaving none of the notes outstanding. The filing gives no reason or funding source. It is a small balance-sheet item next to the quarter’s large numbers, but it shows a managed-care insurer retiring debt early while peers report on margins. Confidence: High on the terms. Source: Centene 8-K.
